Disclosure: Plans and network acceptances may vary. The carrier websites include a cost estimator to allow for a personalized procedure cost estimate based on your specific insurance plan. Please request the procedure codes by contacting the facility or ask for a procedure cost estimate. We strongly encourage patients to contact their health insurer for more information on the total cost after adjustments, deductions and copayments. Providers may or may not participate with the same health insurers or health maintenance organizations (HMO) as the clinical practice which can cause some facility visits to be “out-of-network”. Patients with services rendered at Interventional Rehabilitation Center will obtain a bill directly from the Interventional Rehabilitation Center for facility fees and a bill from the practicing doctor for physician fees.

Patients should contact the health care practitioners anticipated to provide services to the patient while in the center regarding a personalized estimate, billing practices and participation with the patient’s insurance provider or health maintenance organization (HMO) as the practitioners may not participate with the same health insurers or HMO as the center.

As a policy, Interventional Rehabilitation Center doesn’t offer charity cases. Interventional Rehabilitation Center allows for payment plans on outstanding balances on a case-by-case decision. Copays are due on the date of service. Patients are sent multiple statements based on coinsurance balances left by their insurance company. The policy of Interventional Rehabilitation Center is to mail statements to patients; after multiple statements with non-payment, patients may be transferred to a collections agency.
